Monthly:

• $85 for individuals
• $150 for 2 patients from same family or a couple
• $200 for 1 additional family member
• $ 225 for 2 additional family members
• $250  for an entire family (5 or more)

There is a $99 enrollment fee per family and three months membership fee must be paid upfront.
There will be a $50 no show charge if you cancel on the day of your appointment.

Currently not accepting patients over age 64
